St. Mary of the Annunciation Baptism Registration Form

Baptisms are celebrated once a month on the second Sunday of every month at 2PM.  Parents are required to attend a Baptism preparation session held on the Wednesday prior to the Baptism date at 6:30pm.

Baptism Preparation Session Information: The Baptism Preparation session takes place on the Wednesday prior to the Baptism date at 6:30pm. Godparents are welcome but only parents are required to attend.
Please note: Expect a confirmation email within 10 days of submitting this Baptism registration form. If you do NOT receive a confirmation email regarding this Baptism after 10 days, please call us at (978) 774-0340.
Godparent Information: According to canon law of the Catholic Church, at least ONE of the Godparents must be a Roman Catholic who has received the sacrament of Confirmation. Only one Godparent is required. A second Godparent who is a baptized Christian may be chosen as well. If there are to be two Godparents, they must consist of one man and one woman. Please Note: Please be aware no Baptisms are held during the Lenten season. The next Baptism will be held on April 23rd, 2017 and Baptisms will resume the second Sunday of each month following April 2017.
